Reablement care at home

Supporting people within the communities of Clacton, St Osyth and the Tendring Area

Reablement helps people regain their daily life skills and confidence after a period of hospitalisation.



At Distinguished, we want to work with you to determine the core tasks that are significant to you and to set time frames for relearning the skills you need to do these jobs independently. Our aim is to make it possible for people to live complete lives at home and in their larger communities.

Identifying the support you require


Finding out just what kind of support will benefit you the most is our first task.


Our short-term care service's suitability for you will be determined by a social care professional working with assistance from an ECL Trusted Assessor. Your service will typically begin within 24 hours of the social care practitioner making the recommendation.


If you are a patient in a hospital, our Trusted Assessor will pay you a visit and assist you in making plans to return home.




Support and care in your own home


One of our Specialist Care Professionals will visit you at home and undertake with you a personalised support assessment. This implies that we will talk about the service's goals and what you hope to get out of it (your objectives). We use your goals to make sure you have the right support in place and make as much progress as you possibly can.


All of the Distinguished Care Team will work with you in your own home and local community to progress your recovery. We will also assist you to find support systems among your family, friends, and the local community while you are receiving our support.

While the duration of this service is only up to six weeks, longer-term plans will be discussed with you as soon as possible so that you and your family or carers can make future plans.
